Who can provide the letter or note confirming an applicant's diagnosis of lupus?

The diagnosis letter must be confirmed by a physician/M.D. (Rheumatologist, neurologist, nephrologist etc.). It is advantageous to inform the chosen physician (M.D.) that you are applying for a merit-based scholarship when asking for the documentation, as the doctor might write extra information about you.
