2016 L.I.F.E Degree Completion/Scholarship Renewal Program

The L.I.F.E. Degree Completion/Scholarship Renewal Program was designed to help Scholarship winners secure ongoing tuition costs while also providing support and recognition to another college student with lupus! More importantly, it shows commitment to L.I.F.E.’s cause in promoting lupus awareness and academic excellence which is appreciated and deserves recognition as an Honorary L.I.F.E. Scholarship Winner.

To participate in the Degree Completion/Scholarship Renewal Program, all past winners must fulfill the following criteria:

  1. Maintain eligibility requirements for the L.I.F.E Scholarship;

  2. Be enrolled at the same college at which their original Scholarship was received; and

  3. Buy or sell at least one $25 raffle ticket by November 15, 2016 to ensure their place in the program.

  4. Complete and submit this form along with any donations.

FoR Students renewing their Scholarship for the first time

Bronze

This first level gives students the opportunity to receive a $500 L.I.F.E. Scholarship. In order to receive this amount, the student must sell/buy a minimum of one $25 raffle ticket AND raise $600 (the first $100 raised goes to the Foundation to further its cause). A student does not need to raise a total of $600 to receive funds. For example, if a student raises $425, they'll receive a renewed Scholarship of $325 (the Foundation receives $100). Once the student completes the Bronze level, they may move to either the Silver or Gold level.

Students who complete the Bronze Level have the option to move on to either the Silver or Gold level:

Silver

This level gives students the opportunity to receive a $1,000 .I.F.E. Scholarship. In order to receive this amount, the student must raise $1,200 (the first $200 raised goes to the Foundation to further its cause). Like the Bronze level, students raise funds to renew their Scholarship. The only difference between the Bronze and Silver level is that the student has already fulfilled the minimum requirement of selling/buying one $25 raffle ticket so this is not needed to complete this level. A student does not need to raise a total of $1,200 to receive funds. For instance, if a student raises $425, they'll receive a renewal Scholarship at a value of $225 (the first $200 goes to the Foundation).

Gold

For this level, there is no limit to the Scholarship amount a student can receive. Students do not need to complete the Silver level to obtain a Gold level Scholarship renewal. Like the Silver level, students no longer have to sell/buy a raffle ticket and any funds raised after $300, goes to the student’s Scholarship renewal...NO LIMIT! For example, if a student raises $2,500, they'll receive a renewal Scholarship at a value $2,200 (the first $300 goes to the Foundation). This level is intended for students who will raise over $1,200. 

How to raise funds:

  • Sell raffle tickets!

  • Financial donations

    Family and friends can donate on your behalf either online or by sending a check through mail. It is important that your family/friends who donate on your behalf, mention your name on the check memo or the online donation comments section. Past students have found it helpful to email and/or print and mail our current fundraising letter to family/friends informing them that donations received will be applied to their Scholarship renewal.

  • Find a sponsor

    For a minimum of $500 (after you raise the initial $100 for L.I.F.E).

Note: Please do not send cash. All checks sent to L.I.F.E. must be made payable to Lupus Inspiration Foundation for Excellence with your name in the memo and details about the donation i.e. Jane Doe, raffle ticket or Jane Doe, general donation. Please do not hold onto any checks and send them as you receive them (or have your donor mail them directly to PO Box 64088 Tucson, AZ 85728). 

If a student plans to enter the the program, they must raise the funds in the same year that they want to renew their Scholarship. All funds applied to the Scholarship renewal must be received by November 15th of each year. For example, if you start raising money for your Scholarship renewal in 2016, all donations on your behalf must be submitted by November 15, 2016 (post­mark date) so a check can be made out to your institution before December 31, 2016. Failure to provide all donations in a timely manner may result in disqualification or reduced Scholarship renewal funds.

Students must keep track of all donations submitted on behalf of their renewal Scholarship. Students will submit this form that includes the following items (only once you have all donations): 

  • Date: This is the date written on the check or when the online donation was made on your behalf. If you do not know the exact date, the month and year is also acceptable.

  • Name of Donor

  • Donation Amount

  • Donation Details.

It is mandatory to list the number of raffle tickets purchased, if it was a general donation (by check or online donation), and/or if it was a sponsorship.

Students must provide L.I.F.E. with this finalized form by November 15, 2016 (post­mark date or email). Please do not submit forms throughout the year; we only want one final form (multiple pages are acceptable if you need more than one page). Your Scholarship renewal award will be based on the first list we receive prior to the deadline. The L.I.F.E. Scholarship Committee reserves the right to validate all funds received on behalf of the student.
